Activity Forms are used by both members and annual volunteers to provide proof of their volunteer hours each year. Whilst the form is the same for both members and volunteers, the purpose varies slightly:

  • Volunteers are required to submit an activity form to be used as a record for ticket reimbursement and reduced volunteer tickets going forward. 
  • Members are required to submit an activity form totaling 24 hours each financial year to maintain their active status

IMPORTANT! There are 2 vital submission deadlines (although forms being submitted earlier is lovely and preferred!). 

  • Volunteers requiring a REFUND must have their form submitted by June 24th of the year they volunteered. 
  • Members are required to have their form submitted by August 31st each year.
